Abstract

Introduction: The rapid proliferation of electronic cigarettes (e-cigarettes) has outpaced scientific understanding of their long-term health effects. A critical public health question is whether e-cigarette use independently increases the risk of early-onset Chronic Obstructive Pulmonary Disease (COPD), particularly in younger populations and never-smokers, a question complicated by high rates of dual use with combustible tobacco.

Methods: This systematic review synthesized evidence from 80 RCT, etc. Data were extracted on study design, e-cigarette exposure definition, COPD diagnostic criteria (spirometry, ICD codes), control for combustible tobacco use (including pack-years), and risk estimates (OR, RR, aHR). A focused analysis was performed on studies restricting to never-smokers and younger adults (ages 18-50) to assess independent risk.

Results: Significant positive associations between e-cigarette use and COPD/respiratory disease were found. Four major meta-analyses reported pooled ORs for current e-cigarette users versus non-users ranging from 1.48 (95% CI:1.36-1.61) to 1.84 (95% CI:1.51-2.23) (1,2,3,41). Crucially, in never-smokers, exclusive e-cigarette use was significantly associated with COPD (aHR 2.29,95%CI:1.42-3.71)(8) and a pooled OR of 1.44 (95%CI:1.42-1.46)(7). A dose-duration response was identified (aHR 1.28 per year of use, 95%CI:1.16-1.40)(9). Conversely, studies adjusting for combustible cigarette pack-years found no significant independent risk (aHR 1.10,95%CI:0.78-1.57)(4,5). Dual use conferred the highest risk (pooled RR 3.86,95%CI:1.49-10.02)(15). Mechanistic studies confirmed inflammatory biomarker elevation (IL-6, TNF-α) and small airway dysfunction in young users (10-12,32,63).

Discussion: This review reveals a stark dichotomy: the risk of e-cigarettes appears highly dependent on smoking history. In populations with substantial combustible tobacco use, the COPD signal from cigarettes overwhelms any independent e-cigarette effect. However, among never-smokers and young adults, a consistent, significant, albeit smaller, independent risk for COPD and its precursors (respiratory symptoms, small airways disease) exists. The strongest evidence supports that dual use is more harmful than either product alone.

Conclusion: E-cigarette use is associated with a significantly increased risk of COPD and adverse respiratory outcomes, particularly among never-smokers and young adults, representing a novel risk for early-onset disease. A dose-duration relationship supports causality. While switching may benefit established COPD smokers, primary prevention policies must address the independent harm of e-cigarettes to new generations.
